Spring 2025 Reinstatement

The Fall 2024 Reinstatement Form is now closed. Please check back soon for the Spring 2025 Reinstatement Form.

Prior to completing the following form, please have the following applicable items ready for submission:

  • Unofficial transcript(s) of in progress or completed college-level work taken during the out one semester
  • Letter from a medical professional (diagnosis or treatment details not needed)
  • Letter from a work supervisor in cases where established work hours changed 
  • Any other documentation you feel might support your petition to return to CSUSM
  • Acceptable formats for document submission are PDF and Word.

After gathering the applicable items from the above checklist, you will be ready to complete the Reinstatement Form.
